Can I get a note for FMLA, disability, workers' compensation, or a legal dispute?

No. This pathway provides a basic work/school documentation note only. FMLA, long-term disability, workers'-compensation, and legal-dispute documentation require an in-person exam and/or a purpose-specific form that QuickVisitMD does not complete.

How many days can the note cover, and how recent do my dates need to be?

Up to 5 days maximum, and the requested start date cannot be more than 48 hours before you submit your request.

No. This is a documentation-only product — no prescription is implied or issued by the note itself.

How does my employer or school verify the note?
