Is the iris-darkening risk really permanent?

Yes, it can be. Bimatoprost and related prostaglandin medications can permanently increase brown pigment in the iris, especially in people with hazel, green, or other mixed-color eyes. This is why it's important to apply the medication only to the upper lid margin as directed and not let it run into the eye.
